Host Feedback forms

There are two forms that you should get your host supervisor to fill in, they are

  1. DY Host Satisfaction Survey
  2. DY Host Supervisor Assessment Form

Please ensure these are returned to me by your supervisor. Sometimes, hosts return them back to you and that’s fine, just forward them to me. These count for 40% of your DY marks and is an integral component to pass your DY.

OSHE and RA forms

Students undergoing local internship or COP are required to complete OSHE and RA forms before undergoing the internship. The procedure is roughly as follows:

  • Download and fill in the OSHE and RA forms as best as possible.
  • Edit out/delete parts not applicable to your internship and print double-sided if possible to avoid paper wastage.
  • After filling in the forms, you will need to obtain signatures from three parties: 1) one of the DYAs, 2) FOS Dean, and 3) OSHE office (situated at male hostel).
